Changing your appointment

Please give at least 24 hours’ notice to cancel or reschedule. The registration policy lists a $30 charge for missed appointments, same-day cancellations and same-day rescheduling. Read the appointment policy (PDF).

Planning for your visit

Please bring your current insurance card and completed forms, and arrange childcare before your visit. The practice asks that small children do not attend appointments.
